What is a 12-week scan?

The nuchal translucency (NT) scan measures a small pocket of fluid under the skin at the back of the baby's neck. Combined with a blood test, your age and the baby's measurements, it gives a chance estimate for conditions such as Down syndrome. This is known as combined first-trimester screening.

The scan is only accurate within a set window, roughly 11 weeks to 13 weeks and 6 days, so timing matters. The sonographer also checks the heartbeat, early anatomy, the number of babies and the placenta.

Screening gives a likelihood, not a diagnosis. Some people choose NIPT (a separate blood test that looks at the baby's DNA in your blood) instead of or alongside this scan. NIPT is generally not covered by Medicare, and your doctor can help you decide which option suits you.

Preparing for your 12-week scan

  • Book early so the scan falls inside the 11–14 week window.
  • Have the screening blood test done beforehand if your doctor has requested it, usually from about 10 weeks.
  • A partly full bladder may help; follow the clinic's instructions.
  • Bring your referral, Medicare card and earlier scan reports.

A referral from your GP, obstetrician or midwife is needed. The scan and the screening blood test usually attract a Medicare rebate, though many clinics charge an out-of-pocket gap for the scan. NIPT is a separate test that is generally not rebated. The clinic confirms billing when it accepts your booking request.

When is the best time for the 12-week scan?

Between 11 weeks and 13 weeks 6 days. Many clinics aim for 12–13 weeks so the baby is easy to measure.

Do I need the NT scan if I have had NIPT?

Many doctors still recommend an ultrasound at this stage to check early anatomy, the placenta and twins, even if NIPT is done. Your doctor can advise what suits you.

What happens if my result shows a higher chance?

Your doctor will explain the result and options for further testing. A higher-chance result does not mean the baby has a condition.
