What Is Contrast Dye? Safety, Side Effects & Tips

What is contrast dye?
Contrast dye (also called contrast agent or contrast media) is a substance used during some imaging scans to make certain tissues, blood vessels, or organs show up more clearly. It improves the visibility of structures that might otherwise be difficult to see.
Contrast is not used in every scan - your doctor or the imaging clinic will decide whether it's needed based on what they're investigating. When contrast is ordered, it means your doctor wants the clearest possible images to make an accurate diagnosis.
The word "dye" is a bit misleading - it doesn't permanently colour anything. It's a temporary substance that passes through your body and is eliminated naturally.
Types of contrast agents
Different types of contrast are used for different scans:
• Iodine-based contrast (for CT scans): Injected into a vein. Helps blood vessels, organs, and tumours show up clearly. Also used in some X-ray procedures (e.g., angiograms). • Gadolinium-based contrast (for MRI): Injected into a vein. Enhances soft tissues and is particularly useful for brain, spine, and joint MRI. • Barium contrast (for gastrointestinal studies): Swallowed as a liquid or given as an enema. Coats the digestive tract to show up on X-ray or CT. • Microbubble contrast (for ultrasound): Injected into a vein. Used in specialised ultrasound studies of the liver and heart.
The most common type you'll encounter is iodine-based contrast for CT scans.
What does contrast feel like?
When iodine contrast is injected during a CT scan, you may feel:
• A warm or hot sensation spreading through your body - this is very common and completely normal • A metallic taste in your mouth - also normal and temporary • A sensation of needing to urinate - this passes quickly and you won't actually need to go
These sensations last only 30-60 seconds and are nothing to worry about.
Gadolinium contrast (for MRI) typically produces no noticeable sensation. Barium contrast may taste chalky and can cause mild bloating.
The injection itself feels like a normal blood draw - a small needle in your arm with brief pressure as the contrast is administered.
Is contrast dye safe?
Contrast agents are very safe and have been used in medical imaging for decades. Millions of contrast-enhanced scans are performed safely every year in Australia.
However, like any medical substance, there are some considerations:
• Mild reactions (1-3% of patients): Nausea, mild hives, warmth, or itchiness. These usually resolve on their own or with simple treatment. • Moderate reactions (rare): More significant hives, vomiting, or facial swelling. Treatable with medication. • Severe reactions (very rare - less than 0.04%): Anaphylaxis-like reaction. Imaging clinics are fully equipped to manage this.
The clinic will ask about your medical history and allergies before giving contrast. If you've had a previous contrast reaction, tell the clinic - they may pre-medicate you or use an alternative approach.
Who should be cautious with contrast?
Tell the clinic before your scan if you:
• Have kidney disease or impaired kidney function - both iodine and gadolinium contrast are processed by the kidneys. A blood test (eGFR/creatinine) may be required beforehand. • Have diabetes and take metformin - you may need to pause metformin for 48 hours after iodine contrast (your doctor will advise) • Have had a previous reaction to contrast - the clinic will take precautions • Have thyroid disease - iodine contrast can temporarily affect thyroid function • Are pregnant or breastfeeding - contrast is generally avoided in pregnancy; breastfeeding can usually continue (ask the clinic) • Have allergies (especially to iodine or seafood) - while seafood allergy doesn't directly predict contrast allergy, any allergies should be disclosed
The clinic will assess your risk factors and take appropriate precautions.
Tips for your contrast scan
Before your scan:
• Stay well hydrated - drink plenty of water in the 24 hours before and after. This helps your kidneys process the contrast efficiently. • Follow fasting instructions - some clinics ask you to fast for 2-4 hours before contrast CT (to reduce nausea risk) • Bring a list of your medications and mention any allergies
After your scan:
• Drink extra water (1.5-2 litres) over the next 24 hours to help flush the contrast • You can eat and drink normally straight away • You can drive and resume normal activities immediately • If you develop hives, itching, or swelling after leaving the clinic, take an antihistamine and contact the clinic or your doctor
For barium contrast: expect white-coloured stools for 1-2 days and drink extra fluids to avoid constipation.
